Neighbourhood Health Watch and Long Term Conditions

The Neighbourhood Health Watch Team will be at the 3rd Long Term Conditions Network event in January 18th.

 

The focus of this event is on the development of neighbourhood integrated teams. We will be in the ‘Workforce Zone' answering questions, sharing information and learning from what others are doing.
